Currently, our platform only allows the fields that you currently see in the form editor. We plan to update that on our next upgrade, schedule for release in the next 3-5 weeks. If you need a more robust form than we offer, you can use a 3rd-party form builder and embed the form into your website. There are several free form builder platforms out there. Here is a good one to look at: https://www.123formbuilder.com/
